i think you should distinguish between formal and informal way… ‘come si chiama?’ is ONLY informal (come ti chiami? is formal – that’s very important) the same is about ‘come stai? (formal) and ‘come sta?’ (formal)…

“Come si chiama?” is a formal expression; in italian we prefer to say: “Come ti chiami?”.
